Anti-Mage burns her mana with attacks, reflects targeted spells with Counterspell, and can Blink away from danger.

Viper Strike disables her passive and heavily slows her, making close combat and Blink timing much riskier.

Meat Hook can drag Queen of Pain into danger, while Dismember prevents her from Blinking out.

Queen of Pain pressures mid lanes with poison, burst damage, and an elusive Blink escape.
Queen of Pain is a ranged Intelligence hero. Our recommended role is mid.
Queen of Pain is a mobile mid caster who converts lane pressure into quick rotations. Shadow Strike punishes exposed opponents, while Blink changes positioning faster than most enemies can answer. Play around vision, commit for a kill, then leave before return damage overwhelms you.
Use Shadow Strike when the enemy must approach a creep or cannot immediately trade back. Keep the wave near your hill, secure runes with Bottle, and save Blink against likely ganks. Spending Blink for minor damage leaves a dangerous opening for coordinated rotations. Scream of Pain also reflects some hero damage back to her in this snapshot; account for that health cost before a close trade.
Approach from a flank and identify enemy disables before committing. When a safe opening appears, Blink into spell range and aim Sonic Wave through several targets. If enemy catch remains available, cast from farther away and keep Blink for retreat rather than using it to enter.
Cheap attributes and regeneration support the lane; Bottle and Power Treads keep rotations practical. Witch Blade adds attack pressure between spells. Prioritize Black King Bar when control prevents casting, then consider Kaya and Sange for sustained fights, Shiva’s Guard for armor or Scythe of Vyse to catch escapes.

Cast Shadow Strike when an opponent walks forward for a last hit or rune. The slow makes retreat awkward, but do not chase blindly after the poison lands. Keep enough distance to avoid turning a favorable trade into an enemy support rotation.

Wait until dangerous stuns, silences, or roots are used before Blinking into a fight. Blink can dodge incoming projectiles, so watch for a visible targeted spell. A patient Blink preserves both your burst angle and your only reliable retreat.

Line up Sonic Wave so its path crosses several enemy heroes without requiring a deep jump. Consider where the knockback leaves them: pushing a target away from allied melee heroes can spoil follow-up. Cast from range when enemies still have disables ready.
| Detail | What it means in your game |
|---|---|
| Shadow Strike | Poisons an enemy with initial damage, recurring damage and a movement slow. |
| Blink | Teleports a short distance for a new casting angle or an escape. |
| Scream Of Pain | Damages nearby enemies. Part of the damage dealt to heroes is reflected back to Queen of Pain. |
Queen of Pain usually plays mid because levels and Bottle access let her pressure the opposing mid hero, contest runes, and rotate quickly. She needs allies who can start fights or provide vision, because her damage is most reliable against revealed targets.
Witch Blade supports attacks between spells. Black King Bar answers many control-heavy fights, while Linken’s Sphere can stop a key targeted opener. Choose later durability or a Scythe of Vyse for the actual threat; Queen of Pain’s built-in Blink already supplies mobility.
Begin by treating Blink as an escape rather than a damage spell. Harass with Shadow Strike when the enemy commits to a creep, keep Bottle charges ready, and rotate only when a side lane offers a clear target instead of an uncertain chase.
Sonic Wave damages and pushes enemies in front of Queen of Pain across a long path. Use it through several heroes or to finish a fleeing target after their escape is unavailable. It does not replace careful positioning, because casting too close invites disables.
